ASPCA called my vet bill pre-existing and I want to know why

I had been paying for ASPCA coverage for months before I finally needed to submit a vet bill. The claim came back denied as pre-existing. I do not remember my pet having this problem before, so I am confused about which record they relied on. Has anyone asked for the exact notes that triggered a pre-existing decision? I am trying to work out what to request next.
